Sharing Practice – Ryde Cluster

This transfer form was put together by the Ryde area Primary/Middle Liaison Group a number of years ago in response to the need to improve the transition from an assortment of primary schools into the 3 middle schools in Ryde. It gave us a focus for our discussion and a common ground between all primary schools. Also it ensured, before Sims data was transferred electronically, that we received essential data prior to transfer and, more importantly, teacher assessments of pupils. The background information has proved to be invaluable over the years. The form has been modified as tests have changed and new initiatives introduced. It has become a key point of reference for middle school year 5 teachers to turn to during the early days at middle school in particular to check that progress continues and social/behaviour issues are understood. It was created to enhance academic information and provide a picture of the whole child.
